Rhizome Therapy Cooperative is a collaborative, non-hierarchical therapy practice that values the knowledge of both therapists and clients, fostering healing relationships and providing high-quality mental health support.

We offer individual, relationship, and group psychotherapy using the principles of anti-oppression informed practice.

We accept most major health insurance plans including Ohio Medicaid. Self-pay and sliding scale options are also available.

EVIE HEFLIN, LISW-S, LCSW

Evie Heflin is a Licensed Independent Social Worker with Supervisory designation (LISW-S) in Ohio and a Licensed Clinical Social Worker (LCSW) in the states of Kentucky and Indiana. Evie has over 12 years of clinical experience working in community mental health, hospital, and private practice settings.

SAM CONLEY, LSW

Sam Conley is a Licensed Social Worker (LSW) in Ohio. She works with people across a range of ages and life stages, with a focus on those who feel deeply and intensely or feel as though life has dealt them a particularly difficult hand. Sam uses a trauma-informed, person-centered, and strengths-based approach to help clients better understand themselves and feel more grounded in their lives.

NERN OSTENDORF, LISW

Nern Ostendorf is a Licensed Independent Social Worker (LISW) in Ohio. They work with clients of all identities and backgrounds with a focus on LGBTQ+, transgender, non-binary and gender questioning adults. Nern uses anti-oppression and psychodynamic therapy to explore deeper relational, trauma, and systemic issues shaping identity and selfhood.
